Thank you for choosing PT First – Physical Therapy & Rehabilitation for your physical therapy and rehabilitation needs. For our new patients, we ask that you to bring your health insurance card, and physician referral or prescription for physical therapy to your appointment. You will also need to review, complete and initial a series of patient information forms and policy notices. These forms and notices can be downloaded from the links provided below.
On your first PT session, the Physical Therapist perform an evaluation of your condition to determine an optimal treatment plan; this initial session typically takes about an hour. The Physical Therapist will discuss your therapy needs with you and develop a treatment plan for your follow up visits.
Insurance Providers
PT First accepts insurance plans for many healthcare providers. In addition, PT First is proactively working to add other popular providers. Please check the list of insurance plans below to verify that your insurance is listed. We also accept the major automobile insurance providers (e.g., State Farm, Farmers Insurance, etc.) with MedPay coverage. If you do not have insurance coverage, PT First offers competitive self-pay rates. Talk with our Office staff for more information on our self-pay rates.

New Patient Forms
Prior to your first appointment at PT First, please download and print the new patient and medical information forms; the forms can be downloaded in either MS Word or PDF format. Complete and sign the forms, and bring them to your first appointment. If you are not able prepare these forms in advance, no problem! Just plan on arriving at your appointment at least 15 minutes early and you complete the forms then.
Physician Referral Forms
It’s important for you to know that you have the right to choose any physical therapist participating in your plan — just like you can select any physician under your healthcare plan. Neither your doctor or healthcare provider should dictate the physical therapist that you must see. Simply tell your physician or healthcare provider that you want to be referred to PT First. To expedite the referral process, please download the referral form using one of the links below, and provide it to your doctor to complete. Once you have obtained the signed referral from your doctor, please call PT First at (505) 884-4609 to schedule your appointment.
